Class of 2025 College Forms

COLLEGE PROCESSING FORM

  • Please submit to the Guidance Office one form for each college you are applying to.
  • Forms are due in Guidance 2 weeks prior to your application deadline.
  • For January 1st deadlines, processing forms are due no later than November 22, 2024

COUNSELOR LETTER OF RECOMMENDATION REQUEST- Mr. Scherer

  • If you require a letter of recommendation from your counselor, please fill out and submit the request form to your counselor at least 2 weeks prior to when your application is due.

TEACHER LETTER OF RECOMMENDATION REQUEST

  • Request your teacher on Naviance after you personally ask the teacher.

COMMON APP DETAILS SHEET

  • MPHS specific information needed to complete your Common App.

FERPA AND COMMON APP/ NAVIANCE LINK INSTRUCTIONS

  • FERPA Waiver must be completed before we are able to send any of your college materials (transcripts, letters of recommendation).
  • It is highly recommended that you waive your rights; speak with your counselor if you have any questions about this.

 

Matching Your Common App and Family Connection Accounts (step-by-step instructions)

Completing these steps allows your transcripts and recommendations, which are in Naviance, to be matched with your Common App.

First, go to www.commonapp.org:

  • Log into your Common App account. If you haven't created an account, you'll need to do that. Use personal email (not Miller Place email).
  • Under the "Common App" tab, complete the entire "EDUCATION" section so that it has a green checkmark (this is necessary since the Common App must know the high school you attend).
  • Go to “My Colleges.” If no colleges are listed, go to "College Search" and add at least one college to which you'll be applying.
  • Click one of the colleges and check the box next to "Recommenders and FERPA".
  • Complete the FERPA Release Authorization, giving Miller Place High School permission to send your transcript. Follow the instructions and hit save after you digitally sign the FERPA Release Authorization.

 Second, go to Family Connection:

  • LOGIN to Naviance/Family Connection  and click the Colleges tab. Then click “Colleges I'm applying to”
  • Enter the email address used in your Common App account & click “Match”.
  • Once you've done this correctly and you see this, you are finished.
  • "Common App Account Matching -Complete "
  • Your Common App account was successfully match
