• COLLEGE FORMS- CLASS OF 2021

     

    COLLEGE PROCESSING FORM

    • Please submit one form for each college you are applying to.   
    • Forms are due 2 weeks prior to your application deadline for Early Action/Early Decision.
    • For January 1st deadlines, forms are due no later than November 20, 2020.
    • Email forms to: collegeprocesssubmit@millerplace.k12.ny.us
    • Multiple forms can be submitted in one email; must list all colleges in subject line.

     

    FERPA AND COMMON APP/ NAVIANCE LINK INSTRUCTIONS

    • FERPA Waiver must be completed before we are able to send any of your college materials (transcripts, letters of recommendation).
    • It is highly recommended that you waive your rights; speak with your counselor if you have any questions about this.

     

    COUNSELOR LETTER OF RECOMMENDATION REQUEST- Ms. Fiumano, Mrs. Gallo, Ms. Hayden

    COUNSELOR LETTER OF RECOMMENDATION REQUEST- Mr. Scherer

     

    • If you require a letter of recommendation from your counselor, please fill out and submit to your counselor at least 2 weeks prior to when your application is due.

     

    TEACHER LETTER OF RECOMMENDATION REQUEST

    • Please email your request directly to the teacher.
    • Request your teacher on Naviance after you personally ask the teacher.
